The presence of a third vestigial arm is almost always a source of hilarious penalties on social interactions, the details of which are left to the referee.
 
If not affected by a mutagen, a vestigial arm can manipulate or hold items as well as the alchemist's original arms, but it is not strong or coordinated enough to make weapon attacks or wield a shield.  It does allow an Alchemist to swap items from one of her main hands to a vestigial hand as a free action, as per the Base Weapon Rules. (For example, a Vestigial Arm will allow the alchemist to use one hand to wield a weapon, another hand to throw a bomb, and use the third vestigial hand to hold a potion ready for use).
